A Case of Habitual Miscarriage

Case 1:  Files of Doctor Li Shou San - Chong and Ren not Consolidated, Uterus Cold, Blood Stasis Syndrome

Zhu X, female, 29 y/o.  First office call: January 15, 1958. 

1st Visit
After miscarriage lower abdominal feeling of prolapse, distension, cold and pain for 2 weeks.  Miscarried already three times without cause, each time pregnant 3-4 months.  Tried both Chinese and Western Medicine methods, to protect the fetus, with no effect.
Currently, approximately two weeks after 3rd miscarriage.  This time, during the miscarriage, the bleeding was slight and there were clots and abdominal pain.  Today, although the blood has stopped, the lower abdominal feeling of prolapse, distention, cold and pain has not recovered.  Shen is clear, but she is tired.  Complexion is lacking luster, not radiant.  Tongue body is a dim red, fur is thin white, pulse is sunken and stringlike.  With pressing on the lower abdomen there is hardness and pain.  Everything else appeared normal.

Diagnosis:  Slippery fetus (Habitual miscarriage)

Differentiate illness:  Chong and Ren not consolidated, Uterus Cold and Blood Stasis

Tx Method: Warm menses, dispel Cold and invigorate the Blood

Formula: dang gui 15g, chuan xiong 10g, chi shao 15g, chao xiao hui xing (charred fennel) 7.5g, pao jiang (roasted ginger) 5g, rou gui 5g, yan hu suo 15g, wu ling zhi 15g, pu huang 10g (wrap and decoct), yi mu cao 15g, take with yellow wine, water decoction

2nd visit
Jan 20. Took medicine 3 packs, lower abdominal cold and pain stopped.
After menses arrived, still have clots, lower abdominal cold and a feeling of prolapse pulling in abdomen.  Thereby the Uterus Cold Blood stasis is not better.  After taking above formula, 3 packs, all symptoms quickly recover. 

She gets pregnant in April and, after two months, she is experiencing lower back distress and lower abdominal feeling of prolapse.  Because she is afraid of miscarrying again, she came for a diagnosis.

3rd visit
July 6.  4th pregnancy 2 months into it.  Intuits a lower backache and sleepiness, lower abdomen feeling of prolapse, no other complaints.  Her tongue pale red, no fur, pulse appears sunken and fine, cubit position is weak.  The stasis is gone and the Zheng (qi ) is deficient.  Concern is another miscarriage.  Plan is to boost Qi and nourish Blood, mediate and supplement Lv/Kd and consolidate Chong and Ren and fetal source.  Tai shan pan shi san modified

After medicine everything was fine.  Each month she followed the method and took 4-5 packs.  Until seven months, at which point she stopped taking the medicine.  In the winter of that year a baby boy was born after a normal gestation period and natural labor.  Two years later she is pregnant again and, with the same treatment method, she again gave birth to a child.

Note:
Normal calm fetus method: mediate and nourish Lv/Kd and consolidate and contain Chong and Ren.

Also, illness causes Chong and Ren to not be consolidated and damages fetal source.  First, treat illness, then it is correct to calm fetus.  To treat the illness you must treat the root.  When illness leaves the fetus is calm.

This case put to use shao fu zhu yu tang.  This is the method of correctly treating the root to calm the fetus.  This condition depends on the pulse pattern being of blood stasis or qi stagnation.  This is appropriate after miscarriage.  Correct treatment involves moving blood and transforming stasis.  This is the treat the root method.

When the overlying static blood is removed, new blood is calm, which protects the fetus and removes obstruction.  This is the treat the illness to calm the fetus situation. 

After pregnancy, if the blood stasis and stagnation is already gone, then you supplement and nourish Lv and Kd and mediate and rectify qi and blood in order to firm and solidify the fetal source.  To complete the treatment use Shan pan shi san modified.  Consolidate and contain chong and ren, calm the fetus to defend the delivery.  This is the consolidate root to calm fetus situation.

The two consecutive treatment methods resulted in calming the fetus and defending the birth.

When looked at carefully, the patient previously used Western Medicine to protect fetus which was not effective and then used Chinese Medicine to enrich and nourish the Liver/Kidney and solidify and contain the Chong and Ren which was still not effective.
The reason for this is because the stasis and stagnation was not eliminated.

Suppose in this case we did not primarily quicken the blood and transform blood stasis as the root and only used supplement and boost Qi and Blood and enrich and nourish Liver and Kidney to protect the fetus.  I am afraid it would have been difficult to have an effect.
Smooth labor resulted from “Evil being expelled and the Zheng Qi being strong”.  Two years after again a smooth birth.  Mother and child are fine.
