Amenorrhea

Yang Shan Dong case: Zhang XX, female, 32 years old, worker, diagnosed on May 24 1987.  Patient complains of amenorrhea for more then two years.  In 1983, the patient lost her son.   Afterward, the spirit (jing shen) was depressed; she experienced chest stuffiness and frequent sighing.  Eventually, consciousness became muddled.  Furthermore, she experienced palpitations, fear, insomnia, excessive dreams, poor appetite, mouth and lip dryness, her physique gradually becoming thin and finally her menses stopped.  Her tongue tip is dark red, tongue fir is thin, and pulse is thin and rapid.

Then there is emotional depression.  The heart qi is knotted. The ying (construction and yin) is damaged.  The heart fire is excessive.

Treatment is to dredge the liver and undo the depression, support the heart yin, open the heart qi, clear the heart fire and harmonize the blood vessels

Formula: qing xin lian zi yin modified
Qing lian xin, yuan zhi, zhi gan cao each 6 g; bai zi ren, he huan pi, mai dong, huang qin, di gu pi, ze lan, huang bai, niu xi, each 12 g; dang shen, huang qi, sheng shu di each 15 g, xiang fu 9g

She took the above formula successively for 20 packs and her heart palpitations, insomnia and excessive dreams were all alleviated.  Menses started but the volume was small and color dark.  After which she was urged to continue taking the above formula for 10 packs.  Then, she used xiao yao wan and bai zi ren wan alternating for 3 months to consolidate treatment effect.  Interviewed 2 years later her menses was normal.

Note:
This case resembles Li Dong Yuan idea “calm heart, enrich blood, drain fire then menses, by itself moves”.  The formula prescribed, Qing xin lian zi yin modified, boosts qi and yin, clears heart heat, helps the heart and kidney communicate and opens the menstrual pathway resulting in the menstrual flow being regulated.
